3. Complete the 7-part questionnaire (part 3: you do not need to enter insurance information, just select which type and bring your cards with you. (No insurance? No problem!).

We have the Johnson & Johnson (Janssen) single-dose vaccine available at this time.

We receive the COVID-19 Vaccines in multi-dose vials. Once a vial is opened it must be used within SIX (6) hours or it must be discarded. If you must cancel, please provide at least 24-hour notice.
Additionally, do not book multiple appointments, we have many spots open so this duplication is not necessary and causes waste of vaccine for "no-shows" that cannot be filled last minute.
